With the update, the agency consults with its patients’ primary care providers to create individualized treatment and support plans. The company’s staff are specialized in the care of both adults and children with cerebral palsy, spina bifida, and those on the autism spectrum.
Home care services enable people with special needs to maintain independence and receive support in a comfortable and familiar environment. With CHES’s case-managed, specialized nursing services, patients can receive attention and treatment without making a trip to a medical facility. Care plans may include wound care, dressing changes, tube feedings, nutrition monitoring, vital assessments, post-surgical care, pain and symptom management, diabetic care and education, and much more.
In addition, the agency provides two nursing support functions for self-directed service participants. These are Nursing Consultation and Nursing Case Management and Delegation which include Health Risk Screening Tool (HRST) rating or reviewing.
Personal care services involve providing clients with assistance for daily tasks and help to maintain their health and well-being. They may include meal planning, grocery shopping and errands, light housekeeping, assistance with bathing, toileting, dressing, walking and mobility, medication reminders, status reporting to family, and more. Fully screened and certified personal care attendants are matched with clients based on their personality and lifestyle to ensure the right fit.
The agency regularly reviews its patients’ care needs and monitors its quality of service. All staff meet the State of Maryland’s educational and training requirements and are supported to undertake continuous professional development training.
CHES is directed by Dr. Leslie Philips – a specialist in the care of children and adults with physical and intellectual disabilities. Dr. Philips pioneered a comprehensive functional assessment tool to ensure her clients’ needs are met and that they can thrive within their communities. She is joined by Director of Operations, Lisa Noel, MSN, RN, NP-C, who has 26 years of clinical experience in pediatric and adult healthcare.
